fix: move telemetry from hook to inline skill step

fix: move telemetry from hook to inline skill step PostToolUse hooks don't fire for Skill invocations. Move metric sending into each skill's markdown as a bash step that runs first. send-metric.sh now accepts skill name as $1 for direct invocation. Removed the dead Skill matcher from hooks.json.
